Hi there - welcome to planet catfish!
Ancistrus species are notoriously difficult to identify

unless area of capture is known - have you any ideas?

It definitely comes from South America (or technically, possibly from the southern parts of Central America, if we make that distinction).
Unfortunately, without information as to where it came from, it is nearly impossible to put a name to it - that is, if it's a described/known species at all.
However, one thing to consider is that large female
do sometimes "loose" their spots when they get older, and it is not impossible that you have this fairly common species.

Well, blue-ish eyes isn't that unusual in Ancistrus.
I find it very unlikely that the fish would be from French Guyana - there is a reason that there is only a single picture of L045.
As to it being undescribed, that is more of a long-shot again - there are about 100 described species of Ancistrus, and another couple of dozen L-numbers. The Cat-eLog has 132 entries.
The unfortunately part is that we can't really know what it is if we don't have at least some information about where it came from.
Edit: And regarding the size: 150mm total length [TL] corresponds to about 125 mm standard length [SL] (SL => Not counting the actual caudal [tail] fin).

You mean "her name", I guess, as you clearly stated before that it is a female, and I have no doubt that this part is correct.

Well, the real problem is that without a capture location, we would struggle to even guess.
Of course, there are members that have more knowledge than others (Shane, Silurus, Janne to name a few). Some of those do not visit the forum every day, so you may want to give it a few days.
